You enrol a device; its traffic travels through a VPN tunnel to the box, which analyses it in real time before letting it reach the internet.
For the iPhone, the exam goes further: when it's on the same Wi-Fi as the box, the box reads the diagnostics the phone produces about itself and compares them with the known traces of spyware. No app installed on the phone could perform that exam. From the VPN tunnel to that analysis, everything happens on the box, and nothing leaves it.
